Want to take pictures or video during your code club and post those on your social media or website? You should probably have parents fill out a photo release form ahead of time. 

If everything looks good on our template, you can simply download, print, and get parents to to fill out. If you'd like to edit, you'll need a Google account. From there go to File > Make a Copy. This will copy it to your Google Drive where you'll have the ability to make edits. Then you can save and print.
